Job Summary:

The Staff Pharmacist III is responsible for providing professional skills in the distribution of drugs and in clinical services offered to patients and staff. Performs patient and drug therapy assessment and makes recommendations as appropriate for care of our patients. Serve as a preceptor to students.

Major Responsibilities / Duties / Critical Tasks:

·         Interprets physician's orders for medications and ensures therapy is appropriate for patient's diagnosis, weight, and age (pediatric, adolescent, adult, and geriatric). Assesses for contraindications, drug-drug and drug-food interactions, and allergies, appropriateness of drug and dose and therapeutic duplications.

·         Compounds and dispenses prescribed medications per physician's orders in accordance with departmental and applicable laws.

·         Maintains accurate and complete patient medication profiles.

·         Participates in meeting department financial goals through formulary management, inventory control processes and efficient use of labor and resources.

·         Supervises the activities of supportive personnel.

·         Demonstrates and promotes excellence in customer service through teamwork, leadership, decision-making, problem solving and effective communication.

·         Participates in the quality improvement and medication use review activities of the department.

·         Participates in the education and training programs to the pharmacy, medical, and nursing staffs and development of new programs to decrease the cost of care. Provide drug and clinical information to health professionals in response to specific questions or relating to new, evolving issues related to the care of patients.

·         Maintains records and documents all essential information as necessary.
