The Practice Facilitation Manager will supervise a remote field-based practice facilitation team of 10 registered nurses. This team will work directly with Oklahoma providers and clinics who choose to participate in the HMP. The PF manager role is designed to be a community position as it can require clinic and hospital visits and monitoring staff dispersed across the state.
Hybrid Schedule: As field schedule permits, in-office Monday and Friday 9am - 12pm.
Essential Functions
  • Track key performance indicators (KPIs) for the team and individual facilitators, ensuring goals are met.
  • Provide leadership and mentoring while actively participating in the recruitment and facilitation process of each clinic.
  • Conduct regular performance reviews and provide feedback for continuous improvement.
  • Foster a collaborative and positive work environment that encourages growth and professional development.
  • The position requires knowledge of health and wellness trends and best practices.
  • Must have the ability to work with diverse client populations and strong organizational and time management skills.
  • Responsible for weekly, monthly, and quarterly reporting of program activities and outcomes and progress with each clinic's work towards clinical measures compliance and best practices implementation.
  • Participate in weekly leadership meetings and monthly client meetings.
  • Monitor provider satisfaction and community presentations and collaboration with local and state agencies.
  • This position is required to be proficient in working within Telligens proprietary data platform Qualitrac and Johns Hopkins population health analytics software, Care Analyzer.
  • Manage day-to-day operations of the assigned team and participate as a member of the contract leadership team role and responsibilities.
  • Coach, guide, and direct team members to achieve state goals for improving health outcomes and reducing ER utilization and admission.
  • Ensure contract requirements and deliverables are met and submitted as required.
  • Develops and maintains relationships with key stakeholders.
  • Coaches, guides and directs team members to ensure the Management Philosophy that "Managers coach, engage, and support our people to achieve results".
  • Provides resources and support as needed to meet and achieve individual, team, and organizational objectives.
Requirements
  • Current unencumbered RN license in Oklahoma.
  • Four-year degree in nursing, healthcare, business, public health, or a related field.
  • 5 years of experience in a healthcare environment including care management and/or healthcare quality improvement.
  • 3-5 years managing projects and/or a professional staff.
  • Demonstrated experience working in a fast-paced and deadline-driven environment.
  • Medicaid contracting expertise
  • Proficient in Microsoft products
  • Strong work ethic
  • Innovative
  • Quality assurance and data analytics experience
  • Ability to establish positive relationships with stakeholders both external and internal.
  • Confidence and skill in pitching, presenting, reporting, and recruitment efforts with hospitals, clinics, providers, and community Stakeholders.
  • Self-driven and motivated
