PATH Outreach Counselor

Riverbend Community Mental Health, Inc., one of the largest providers of community-based behavioral health services in New Hampshire, is seeking a full-time PATH (Projects for Assistance in Transition from Homelessness) Outreach Counselor.

As a PATH Outreach Counselor, you'll be working to promote hope and recovery by assisting homeless or at-risk Riverbend or PATH consumers in the management of symptoms associated with mental illness or a co-occurring substance abuse disorder, through the use of rehabilitative services and environment supports.

Responsibilities:
  • Provide outreach services, to include case management to non-Riverbend individuals and Functional Support Services to eligible Riverbend individuals who are homeless or at imminent risk of losing their housing.
  • Assess and screen individuals for mental health and substance use services in the field; refer to appropriate agencies based on individual's needs.
  • Assist homeless individuals in applying for income support services and affordable housing.
  • Record and enter data into Homeless Information System, reporting outcome measures quarterly and annually to the State PATH contact.
  • Work collaboratively with other state PATH outreach workers and attend quarterly trainings/meetings.
Qualifications:
  • Bachelor's degree in Psychology, Social Work, Human Services or related field is required.
  • 2 years of related experience working in the field is preferred; work with the homeless and/or substance use population is also preferred.
  • Knowledge of the mental health system and homeless provider network is preferred.
  • Working knowledge of HMIS (Homeless Management Information System) is preferred.
  • Valid driver's license, personal vehicle and auto insurance are required. About Riverbend
