About this Role

This is an opportunity for a mission-driven team player who is passionate about delivering high quality, integrated care to frail older adults. In this role, you will be responsible for the management and delivery of direct nursing care to PACE (Program of All-inclusive Care for the Elderly) participants in a variety of settings including, but not limited to, the clinic, adult day center, home, virtually, and beyond. You will serve as a critical member of the Interdisciplinary Team (IDT) and work collaboratively to provide high quality nursing care and drive forward participant care plans. This role will allow you to practice at the top of your license, promoting ongoing learning and development of professional expertise.

Expectations for this Role
    • In partnership with a medical provider and a clinical case manager RN, performs requested tests, medication administration, treatments, nursing interventions, and delivers personalized care for a panel of participants in the clinic, day center, or home setting.
    • Communicates and collaborates with clinical and interdisciplinary team members to help participants achieve their goals while operating within their scope of license.
    • Performs and documents nursing interventions based on care plans and/or provider orders.
    • Observes and records signs, symptoms, interactions, and progress of participants and promptly reports findings to the medical provider and clinical case manager RN.
    • Participates in education of participants and caregivers including family members as assigned through established care plan and protocol.
Attributes of an Ideal Candidate
    • Passion and mission orientation for serving high-risk seniors and frail older adults.
    • Minimum 2 years of experience clinically caring for medically complex or older adult populations preferred.
    • Active listening and conflict management skills
    • Adapts quickly to multiple clinical settings.
    • Strong clinical acumen in providing clinical interventions to complex participants.
    • Demonstrated experience in wound care, IVs, phlebotomy, colostomy/ileostomy care, etc.
    • Active, unrestricted LVN license in California
    • CPR and First Aid certification (within 60 days of employment)
    • Alzheimer's certification is preferred but not required.
    • Languages Spoken: English and Spanish preferred.
    • Proof of current CPR/BLS certification required or requirement to obtain one within 30 days of being hired.
    • Proof of a valid CA driver's license, personal transportation, good driving record and auto insurance as required by State law
