Job Description:Responsible for executing all aspects of bed management and transfer center operations in a safe and timely manner, in accordance with standard operating procedures and regulatory requirements, scope is inclusive of patient access, placement, transfers and bed management. Uses clinical knowledge, critical thinking and communication skills to support the Command Center in providing a reliable, high-quality service that allows referrals, transfers, and other healthcare settings to be accepted in an efficient and timely manner. ESSENTIAL DUTIES AND RESPONSIBILITIES Assesses, directs and coordinates transfers and admissions from physician offices, hospitals, and other healthcare settings to the most appropriate accepting service, facility, and level of care. Facilitates patient placement by assessing patient diagnosis, acuity and location, applicable risk factors and unit criteria/bed availability. Collaborates with the facility departments, medical staff, nursing staff, and leadership to promote appropriate, timely patient placement so that patient care is not delayed. Collaborates with facility staff to identify and communicate bed capacity issues. Leverages capacity dashboards and other real-time indicators to predict future patient placement pressure points and participates in the execution of surge-capacity protocols; and initiating interventions, within scope of practice, to accelerate patient flow or other mitigating barriers to optimal throughput. Assists with all aspects of patient logistics to include pre-registration, transportation, and bed planning; collects unit specific data, and information needed to access and place patients appropriatel Adheres to regulatory compliance with all Command Center operations, including Stark, EMTALA, and any other pertinent to Call Center activities. Maintains current knowledge of regulatory requirements impacting operations of the Command Center. Provides timely and accurate transfer and admission documentation. Works collaboratively with ourproviders and key stakeholdersin the development and maintenance of transfer and admission pathways for our patients. Delegates as needed to the Transfer Center Tech and EMS Liaison. Promotes mission, vision, and values of SCL Health, and abides by service behavior standards. Performs other duties as assigned. May be required to be placed on-call during a regularly scheduled shift. EDUCATION Required: Bachelor's degree of Nursing (BSN) Preferred: CERTIFICATION, REGISTRATION, LICENSE (*indicates primary source verification requirement) Required: Current license as a Registered Nurse in the state of practice is required. Current American Heart Association Basic Life Support for Healthcare Providers (BLS-HCP) certification upon hire or obtained within 60 days of hire with prior approval.. Preferred: ACLS. A specialty certification (CCRN, PCCN, MSRN, or CEN). EXPERIENCE Required: Minimum 5years RN experience in either ED, ICU and/or 5 years acute care inpatient nursing. Preferred: Critical Care or ED background. Hospital Supervisor, Charge Nurse or previous transfer center experience. Experience with hospital throughput or capacity management. Experience with Command Center operations, knowledge of Epic. KNOWLEDGE, SKILLS, AND ABILITIES To perform this job successfully, an individual must be able to perform each essential duty satisfactorily.
