Nurse Care Coordinator Job Description

Philadelphia FIGHT was initially founded in 1990 as a comprehensive AIDS service organization. Today we are a Federally Qualified Health Center (FQHC) with clinical sites that provide state-of-the-art culturally competent primary care, advocacy, social services, consumer education and outreach for patients in need regardless of HIV or insurance status.

The patients and staff at Philadelphia FIGHT are diverse and represent a broad spectrum of race, religion, gender identity, sexual orientation, HIV status, and physical ability. All staff are required to consistently demonstrate sensitivity to the diverse nature of our community. Successful FIGHT staff are professional, innovative, emotionally intelligent, intellectually curious, hard-working, reliable, and committed to a trauma-informed, patient-centered approach in all interactions with patients and their families.

Qualifications: A degree in nursing is required and a Registered Nurse license for the state of Pennsylvania. We are looking for nurses that have a general knowledge in science, medicine, and the running of a medical clinic. Can demonstrate the ability to understand the overall purpose of clinical care and the importance of support staff. Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health counselors. Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in a multi-task environment. Needs to have attention to detail, dependability and flexibility.

Responsibilities include:

Clinical Responsibilities (0.75 FTE)

  1. Triage sick/emergency walk-ins.
  2. Triage sick calls.
  3. Communicate normal and abnormal labs to patients and recommend follow up based on medical provider recommendation.
  4. Assist with one-on-one management and teaching of complicated patients with multiple diagnoses, complicated medical regimens, and/or complex social needs.
  5. Identify and advocate for patients needing urgent or ongoing behavioral health services and/or substance abuse treatment via collaboration with the primary provider and/or onsite Behavioral Health Consultant.
  6. Administer therapeutic injections and vaccinations and perform phlebotomy when needed.
  7. Perform clinical tasks: i.e. EKG, oxygen setup, blood glucose monitoring, urine dipsticks and pregnancy testing as backup to clinical medical assistant.
  8. Perform CLIA related tasks such as running of controls as back up to nurse manager.
  9. Coordinate ordering of clinical supplies, including monitoring for expiration of supplies and maintenance of clinic medical equipment (e.g., medical supplies and prescriptions).
  10. Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, and behavioral health consultants.
  11. Complete all charting and documentation as needed and required by contracted funding sources, including assessments, progress notes, and administrative documentation.
  12. Complete all billing documentation as needed and required by contracted funding sources, including billing codes and required documentation.
  13. Demonstrate commitment to clinical care and confidentiality.

Administrative Responsibilities (0.25 FTE)

  1. Assist providers and the nurse manager with obtaining urgent reports from diagnostic exams, specialist consults, and hospitalizations along with discussing complicated care patients with specialist offices.
  2. In conjunction with medical assistants, assist providers and nurse manager, in the scheduling of imaging studies, procedures and specialty appointments, particularly those requiring prior authorization.
  3. Track high risk and complex patients using excel spreadsheets (i.e., patients on Coumadin or those that are pregnant, etc).
  4. Order and coordinate delivery for durable medical equipment.
  5. Assist medical providers in the completion of essential clinical paperwork and in the generation of letters of medical necessity.
  6. Report and assist in communicating appropriate information on reportable diseases to oversight bodies, e.g., Public Health Department, AIDS Activities Coordination Office.
  7. Coordinate ordering of clinical supplies, including monitoring for expiration of supplies and maintenance of clinic medical equipment (e.g., medical supplies and prescriptions).
  8. Assist Nurse Manager in the monitoring and reconciliation of orders in the EMR.
  9. Interact closely with the other clinicians in the practice, Practice Coordinator and Administrative Director, by participating in regular meetings for purposes of addressing management issues and quality assurance.
  10. Attend all Lax team meetings and attend FIGHT case conference weekly.
  11. Alongside other Lax staff, participate in regular cultural competency trainings to best serve our diverse patient population.

Physical Demands: Occasional lifting of 10-20 lbs. and pushing of 5-20 lbs. Sitting approximately 80% of the time. Normal accessibility and mobility required. Ability to work at a computer screen for long periods of time.

Work Environment: Generally, works in a high-volume, clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting. May also be required to provide coverage at all Philadelphia FIGHT Community Health Centers.

Other: Initial probationary period is three months. Performance of other tasks may be assigned on an as-needed basis.
