Reports to Pharmacy Director, or designee. The Outpatient Pharmacist is responsible for optimizing patient care outcomes through interdisciplinary medication management. This is primarily accomplished through interpreting prescriptions and the preparation, dispensing, and monitoring of medications used in a community pharmacy. The pharmacist is expected to sustain an environment for the safe, rational, effective, and efficient use of medications prescribed to patients of all age groups. Pharmacists are responsible for the direct supervision of pharmacy technicians and students. All job duties will be performed in accordance with all applicable Indiana and Federal laws, applicable regulatory agency standards, professional practice standards, and policies and procedures of the health system and pharmacy department. Dispense/Profile Review:
  • Reviews all prescriptions for completeness, accuracy, interactions, appropriateness and proper dosing, and dispenses medications and supplies to patients accurately.
  • Utilizes the double check system at all times.
  • Checks all prescriptions, correcting any errors to ensure accuracy.
  • Calls insurance companies to facilitate successful prescription claim processing and prior authorization requests.
  • Ensures compliance with third party dispensing requirements (day's supply, DAW codes, prescription source code, etc.) and formulary maintenance of the individual insurance plans, if possible.
  • Utilizes the software systems to maximize workflow efficiencies.
  • Works within standard work to ensure safe and efficient workflow; uses barcode scanning technology capabilities within the Pharmacy Information System to ensure the highest levels of patient safety.
  • Electronically transmits controlled substance records to the INSPECT program per Board of Pharmacy regulations.
  • Oversees the dispensing of all C-II medications and maintains all records including inventory counts and any other required records according to federal and state regulations. Complies with all record keeping requirements for controlled substances.
  • Proactively seek ways to individually prevent medication errors and ways to improve department processes related to medication dispensing.
  • Maintains complete and accurate patient profiles checking for drug interactions and allergies prior to dispensing medications.
  • Fills prescriptions following all federal and state laws and regulations pertaining to dispensing of drugs and controlled substances.
  • Oversee audit of prescriptions waiting to be picked up by patients and return to stock any that have not been picked up after 14 days, processing the return in the Pharmacy Information System computer, reversing any insurance claim transmission.
Patient Counseling/Clinical:
  • Answers all drug information questions and provides appropriate counseling based upon the patient's needs, including questions related to OTC items.
  • Ensures compliance with OBRA regulations regarding counseling requirements demonstrating a high level of customer service skills when interacting with the patient.
  • Demonstrates ability to meet the needs of all patient types (i.e., pediatric, neonatal, adolescent, geriatric, renal compromised, etc.) when reviewing prescriptions and intervening with physicians to clarify prescriptions for identified changes.
  • Participates in clinical therapeutic intervention programs by documenting calls to physicians for clarifications and/or order changes or other issues on the hard copy of the prescription.
  • Regularly reviews patient profiles and makes suggestions for therapeutic alternatives (streamlining of therapy), generic alternatives, or formulary substitutions that would benefit the patient or be cost effective options.
  • Assists identified patients by referral to the Medication Assistance Program or by identifying manufacturer's coupon programs that may be used.
  • Politely answers all phone calls (including name and credentials) so that patients have an excellent customer experience.
  • Triage customer phone calls and window visits so efficient department workflow is maintained.
  • Provides Medication Therapy Management (MTM) services, as appropriate.
Accounting/Inventory:
  • Ensures accurate patient charging which is based on approved pricing schedules.
  • Analysis of pricing formulas on a regular basis. Maintenance of up-to-date price files in Pharmacy Information System.
  • Oversees technician staff in ensuring that daily cash register deposits, daily cash reports, and daily sales analysis reports are completed and forwarded to the Cashier and Finance Department daily.
  • Oversees technician staff in ensuring completion of all third party reconciliations.
  • Provides Monthly Reports to Finance Department.
  • Applies price updates from drug wholesaler to Pharmacy Information System.
  • Assist with monitoring contract pricing.
  • Oversees the ordering and quality of inventory to meet customer demand, minimize costs and maximize profitability. Proper inventory par levels are followed.
  • Coordinates the receiving and storing of medications and processing of returnable goods for credit.
  • Oversees the ordering and invoice coding and processing process in the P2P electronic system, if applicable.
  • Oversees the completion of annual department inventory and bi-annual controlled substance inventory.
  • Processes drug recalls as received from wholesaler or drug manufacturers.
Operations:
  • Cooperates with other hospital departments and physician offices to provide pharmaceuticals such that patient needs are met (i.e., special needs, HR Advocacy, medics, vouchers).
  • Coordinates and oversees the work performed by the pharmacy technicians, assisting with technician duties to maintain work flow.
  • Assists in the development of policies and procedures for the Outpatient Pharmacy.
  • Oversees the checking of supplies and medications for outdated items on a monthly basis.
  • Participates in quality improvement for the department or hospital. Ensures all required quality logs are maintained, such as refrigerator temperature and compounding logs.
  • Ensures compliance and readiness with regulatory agency standards.
  • Completes special projects, as assigned.
  • Participates in the orientation of new staff members, orientation is performed thoroughly and cordially.
  • Assist with third party audits, patient medical records requests ensuring HIPAA regulations are met.
  • Assist in marketing efforts such as brochure and advertisement creation.
  • Works with Pharmacy Management to improve operations, sales, and budget maintenance, quality and performance goals.
Contribute to the overall effectiveness of the department:
  • Completes other job-related duties and projects as assigned.

  • Direct patient care providers are required to maintain current BCLS (CPR) and other certifications as required by position/department.
  • Consistently utilizes appropriate universal precautions, protective equipment, and ergonomic techniques to protect patient and self.
  • Adheres to regulatory agency requirements, survey process and compliance.
  • Complies with established organization and department policies.
  • Available to work overtime in addition to working additional or other shifts and schedules when required.

Education and Experience
  • The knowledge, skills, and abilities as indicated below are normally acquired through the successful completion of a Bachelor's Degree, BS Pharmacy or PharmD. A valid Indiana pharmacy license or licensure eligibility is required. Retail pharmacy experience preferred with strong background in third party insurance plans.
