Employment Type: Full time Shift: Day Shift Description: GENERAL SUMMARY The sepsis coordinator is a health care professional who works in collaboration with the medical and hospital staff to provide multidisciplinary care to the septic patient. The coordinator provides clinical, professional expertise for patients, families and staff members, facilitates the bedside clinical care of sepsis to assure that all patients receive comprehensive and timely treatment interventions, consistent with national and St. Joseph Mercy Hospital’s best practice recommendations. This position collaborates with all members of the healthcare team through consistent daily clinical patient rounds to assist in the planning, implementing, monitoring and evaluation of care delivery for sepsis patients. REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE Master's Degree required. Current licensure by the State of Michigan as a Registered Nurse (BSN/MSN or APN preferred). BLS certified. Minimum of 3 to 5 years of emergency or critical care experience with progressively more related work experience in quality and safety activities. ESSENTIAL DUTIES AND RESPONSIBLITIES Works under the direction of the Sepsis Program Quality Leader, and the Sepsis Collaborative Team in collaboration with the Department of Nursing, Nursing Education Department, physician leadership, and Marketing Department to facilitate program projects and sepsis education. Facilitates the implementation and evaluation of the Sepsis program including all systems necessary for the multidisciplinary approach throughout the continuum of care. Makes regular rounds on sepsis patients to evaluate appropriateness of orders, treatment plans, nursing intervention, physician documentation and compliance with the Sepsis bundle to assure that all patients receive comprehensive and timely treatment interventions that are consistent best practice guidelines . Utilizes currently available reports to identify sepsis cases and facilitates data collection process and assesses and analyzes outcomes. Recommends additional reports as necessary. Collaborates with frontline staff to identify on-going care concerns related to sepsis care and works with them to develop strategies for improvement. Collaborates with leadership and colleagues in identifying sepsis quality of care issues, in order to reach collaborative action plans and outcome evaluations. Determines baseline compliance with physician documentation and compliance with the Sepsis bundle. Provides real time and detailed feedback to all clinical providers and departments and scheduled updates to the Sepsis Collaborative Team and work groups. Assist the rapid response team and other hospital staff, when necessary, if dealing with a patient situation in which there are obstacles preventing the appropriate sepsis treatment from being provided. Facilitates close multidisciplinary partnership between the ED, ICU, PCU/MS, and other departments. Co-Chairs the Sepsis Collaborative Team and serves on other work groups, as needed. Along with the Sepsis Collaborative team, facilitates defining outcome and process measures of the Sepsis program for the severe sepsis and septic shock population such as, improvement in Compliance with the Surviving Sepsis Campaign Resuscitation and Management Bundle, reduction in LOS Reduction, reduction in the mortality rate, reduction in sepsis related readmissions. Conducts sepsis organizational tracers to identify quality and safety issues. Analyze data to identify trends and issues, also use improvement tools to assist with problem solving and action planning. Assist the concept of “making it easier to do the right thing” by supporting the development of the EMR such as, RN Screening tools, MD Order sets, MD documentation, and the identification of additional opportunities. Provides oversight and support for the core measure data abstractors related to the sepsis measures. Collaborate with clinical documentation specialists to help with appropriate documentation to optimize coding. Provides formal and informal education to medical and clinical staff. Utilized Six Sigma and other process improvement tools to improve sepsis care and outcomes. Provides formal and informal education to medical and clinical staff. Collaborates with nursing management and the education department, and Medical Staff to develop and implement an education model based on current educational resources and structures in place. Maintains knowledge of current trends and developments in the sepsis management, fields of quality, and safety by reading appropriate literature and attending related seminars. REQUIRED SKILLS AND ABILITIES Experience with and understanding of key functions of hospital and medical staff departments or services. Advanced interpersonal skills necessary to effectively communicate complex clinical and technical concepts effectively with a wide variety of individuals. Advanced analytical and computer skills in order to manage project work and data. Advanced analytical skills necessary to develop sound recommendations and assist in implementing improved delivery of care systems, policies and procedures, evaluating clinical practice outcomes and developing educational programs. Ability to work with and lead multidisciplinary teams in initiatives to facilitate implementation and measurement activities. Ability to effectively identify, assess, and facilitate resolution of any conflict which may arise and to adjust project scope appropriately. OTHER FUNCTIONS AND RESPONSIBILITIES Participates actively with leadership in recommending decisions regarding practice issues and material resources. Present data to Quality and other hospital and medical staff committees (including hospital board). Uses and replicates research relevant to the needs of the patient care environment. Work with other system sepsis leads (Chelsea, Livingston, Oakland and Livonia) as developing sepsis program through Trinity Sepsis Collaborative.
