We are actively seeking a highly motivated full-time, exempt, Licensed Clinical Social Worker or Licensed Social Worker to join our Resident Health Services team at Monarch Landing. Our Life Plan Community supports residents as they age in place. The Social Worker/Navigator is a pivotal member of the team and is responsible for coordination of care, navigation through transitions of care, and provide meaningful interventions along the way. What you will do...

The goal of the Social Worker/Navigator is to guide residents, family members, and/or caregivers through successful health and wellness transitions to achieve the optimal level of well-being and appropriate level of care. The Navigator provides relational services by building connections with the residents, gaining trust, and providing emotional and psychosocial support through the aging process. This person will ensure a seamless transition between appropriate levels of care offered at Monarch Landing. The Navigator will lead support groups, be the liaison for the medical clinic, and work closely with the Resident Services team.
