To thrive as a Full-Time RN at Comfort Home Care, you'll need a diverse skill set that highlights your expertise in home health nursing. Your ability to conduct initial and ongoing assessments is crucial for identifying clients' medical needs and communication abilities. You'll initiate and revise care plans while performing OASIS assessments at the right moments, showcasing your strong critical thinking skills. Coordinating services and promptly informing physicians of any changes in clients' conditions requires excellent communication and collaboration abilities.

Moreover, you will work closely with patients, families, and other health care providers to craft tailored care plans and seamless discharge processes. Your proficiency in preparing clinical and progress notes, alongside timely paperwork submissions, will ensure that our high standards of excellence are met. Knowledge and skills required for the position are:

  • Provides initial and ongoing assessments to identify the client's medical needs level of functioning and ability to communicate and learn.
  • Initiates the plan of care necessary revisions and performs OASIS assessments at appropriate time points when applicable.
  • Initiates and performs appropriate nursing interventions.
  • Coordinates services informs the physician and other personnel of changes in the client's condition and needs.
  • Collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
  • Prepares clinical and progress notes and submits all required paperwork in a timely manner according to agency policy and procedure.
