Job Description Summary:

This position performs patient care by assessing the patient's physical and functional abilities. Based on problems determined, the physical therapist establishes and carries out a plan of care to include patient/family instruction, discharge planning, documentation on the medical record, and communication with physicians and other medical staff. This position helps patients achieve maximum level of physical functioning and acts as clinical instructor for students and actively participates in department functions.

Minimum Qualifications:

Bachelor's Degree (Required)BLS - Basic Life Support - American Heart AssociationAmerican Heart AssociationAmerican Heart Association, DL - Driver's License - Department of Motor VehiclesDepartment of Motor VehiclesDepartment of Motor Vehicles, LIC - Licensed in Field - State of OhioState of OhioState of Ohio, PT1 - Physical Therapist License - American Board of Physical Therapy SpecialtiesAmerican Board of Physical Therapy SpecialtiesAmerican Board of Physical Therapy SpecialtiesState Driver's License. Licensed in the State of Ohio; CPR Certification . Basic computer skills. Entry-level new graduate, if facility has at least one veteran Physical Therapist to mentor, otherwise, at least one year of clinical Experience .
