JOB SUMMARY:

The Community Health Advocate (CHA) is part of the Hospital's Behavioral Health Emergency team that provides a screening and assists with disposition plans for adult and pediatric patients that present in the ED with a mental health crisis. This position helps behavioral health patients connect with resources that allow them to better manage their mental illness, improve their overall health and avoid preventable hospital encounters. The CHA is an integral part of the multi-disciplinary team that includes the ED staff and psychiatric Social Workers. The CHA embedded in the ED is responsible for assisting with post discharge care and connecting patients to community resources and supports for individuals with complex needs. This position also provides post-discharge follow up with patients seen in the ED for mental health crisis in order to promote and support participation in referrals for outpatient treatment and other community-based service referrals made while in the ED.

EDUCATION & EXPERIENCE:

  1. Bachelor's Degree with an interest in behavioral, community or public health.
  2. Experience working in a hospital or clinic setting preferred.
  3. Experience working as part of a multi-disciplinary team as well as with people of diverse backgrounds preferred.

LICENSES & CERTIFICATIONS:

  1. Must obtain CPR certification within sixty (60) days of hire and maintain thereafter.

MAJOR JOB RESPONSIBILITIES:

  1. Conducts screening with patients to determine level of need and support post discharge for success with participation in referrals made Social Workers and the psychiatric team
  2. Makes referrals to community-based programs and services. Attempts to ensure warm handoff to services upon discharge (e.g. appointment made prior to discharge)
  3. Works with ED staff, social work and case management to identify individuals discharge needs and provides patients with education around programs and services available in the community
  4. Assists patients with scheduling and finding transportation to medical appointments or community-based behavioral health appointments
  5. Maintains extensive knowledge about community resources appropriate to needs of patients including self-help programs, transportation and social supports
  6. Completes timely and accurate documentation of patient interaction and referrals.
  7. Provides patient education on use of appropriate crisis resources as an alternative to ED utilization
  8. Conducts follow-up with patients through providing telephonic basic motivational interviewing and goal setting with patients/families around achieving treatment needs
