Qualifications: Masters in Social Work, Counseling, or related certification
Licensure in Social Work or Counseling
Prior experience working with at risk-youth and families
Strong background working with schools and community agencies
Excellent communication skills, written and oral
Function: Facilitate programs and services to schools, youth, and families for the purpose of promoting positive school attendance. Coordinate efforts with schools, community agencies, county departments and local law enforcement through specific interventions for referred youth, and the facilitation of best practice strategies.
Reports to: Truancy Team Lead
Employment: Full Time, 180 Days, Salary $50,000
Description of Tasks and Responsibilities
- Oversee and connect services for truant and chronic truant students and families, meet regularly to review cases, schedule services, and coordinate items requiring legal support and court intervention
- Collaborate with local school districts and community/county agencies to provide prevention/intervention programs, inclusive of Attendance Improvement Mentoring (AIM), based on best practice strategies for working with at-risk youth and families
- Coordinate and implement an Individualized Optional Education Plan (IOEP), diagnostic profile highlighting student needs, goal setting, case monitoring, connection to community resources, home visits, life skills training and referral for social/academic services
- Develop and facilitate communication and training with identified representatives from partnership schools, juvenile justice system, community agencies, county programs to promote and inform regarding services provided; identify current community needs and issues for students served; explore and seek best practices and resources to serve students, families and schools of DuPage County.
- Maintain current data and progress monitoring related to reporting, both local and state; contributing to annual grant submission (TAOEP) promoting data driven decision making
- Participate in a Professional Learning Community (PLC) promoting collaboration, reflective processing and ongoing professional learning
- Other duties and assignments, as directed
