POSITION TITLE: Case Manager, Connections to Care

REPORTS TO: Behavioral Health Manager, Mental Health/SUD Diversion

LOCATION: Woodland, CA

Salary Range: $22.37-$28.22 hourly

JOB SUMMARY The Case Manager is part of a multidisciplinary team consisting of mental health clinicians, peer support advocates, and management team members. The Case Manager utilizes a client-centered approach and collaborates with each client on their unique stability, resource and wellness needs. The Case Manager demonstrates an ability to engage in trauma-informed and responsive service delivery and uses and models destigmatizing and strength-based language with clients and partners alike. Case Manager understands and/or develops an understanding of the Social Determinants of Health and systemic inequalities that impact wellness, quality of life, and stability. Case Manager works to link clients to critical resources, benefits, and supports

EDUCATION, EXPERIENCE, TRAINING

  1. Experience working with those having involvement in the justice system and who are experiencing a range of mental health conditions including those whose conditions are severe and/or are experiencing substance use disorders preferred.
  2. Bilingual English/Spanish preferred
  3. Must have current/valid drivers license and clean driving record.


KNOWLEDGE OF WORK

1.

Experience or knowledge of re-entry services, diversion programs, restorative justice, and/or harm reduction strongly preferred.

DUTIES AND RESPONSIBILITIES

1.

Provides in-person services including needs assessment, crisis support for clients experiencing a range of mental health symptoms and/or substance use disorders. Creates case/care plans in collaboration with client.

2.

Collaborates actively with physical and mental health providers, including Yolo County Health and Human Services Agency and legal partners as necessary and with client permission to do so.

3.

Support clients with care coordination, referrals, and linkage to resources, benefits, vital documents, and community supports.

4.

Ability to accurately and quickly attune with clients, respect client choice, build trust, and identify needs. Works with client to reduce barriers to program engagement and support low-barrier access to care.

5.

Manages complex caseload during a fast-paced workday and face-to-face service requirements for program

6.

Effectively utilizes measurement-based care practices to determine client progress, wellness and risk factors, collect required departmental screenings as directed by contract and/or agency.

7.

Completes documentation within 48-hours of services in accordance with working hours, all documentation must comply with Medi-Cal, Medicare, Partnership, Carelon if indicated. Participate in peer chart reviews and other quality improvement efforts.

8.

Complies with established department policies and procedures, objectives, quality assurance program, safety, environmental, and service standards.

9.

Based on the individualized client assessment, develops a plan for the provision and services, in collaboration with the client with measurable goals.

10.

Participates in team-based approach to care including staff meetings, case review, consultation/supervision sessions and other forums as required.

11.

Creates a supportive discharge or program completion process, in collaboration with other members of the team.

12.

Responsible for remaining within established ethical standards, including those of confidentiality and mandated reporting.

13.

As safe and appropriate to do so, transport clients or support with transportation arrangements.

14.

Attend trainings/conferences as arranged.

15.
