Case Manager

The Case Manager is responsible for providing direct service to clients, conducting process and psycho-educational groups and for facilitating the smooth day-to-day operation of the program. The Case Manager collaborates as a member of the multidisciplinary team (Treatment Team) to deliver effective treatment.
Essential Duties and Core Competencies:
- Support clients in accessing community resources
- Make appropriate referrals for clients to other services in coordination with the Clinical Team
- Conduct process and psychoeducation groups
- Complete documentation in EMR
- Coordinate with other treatment providers
- Demonstrate appropriate boundaries with clients
- Supports clients by utilizing outcome measures
- Assist Clinical Director with Community Care Licensing requirements
- Demonstrate effective crisis intervention
- Teach independent living skills effectively
- Utilize individualized approach to client services
- Support clients who have discharged with follow up calls
- Advocate for clients
- Participate in staff meetings
- Utilize supervision with the Clinical Director to enhance clinical skills
- Masters Degree preferred.
- ACSW a plus
- One year full-time experience working directly with clients in a program serving people with mental illness.
- One year of Case Management experience in a mental health or substance inpatient or residential setting.
- Knowledge of and belief in the principles of client centered treatment.
- Knowledge of medications and interventions preferred.
- Demonstrated ability to work independently and as a team player.
- Excellent communication skills both oral and written.
- Able to maintain a professional manner with clients, staff, clients and visitors.
- Able to work in a fast-paced setting, under pressure and maintain open, and proper communication.
- Demonstrate professionalism and strong boundaries with the clients.
- Must be fluent in speaking, reading, and writing English.
- Current AMFT, ASW, APCC or equivalent Preferred.
- Valid driver's license, clean driving record and personal automobile insurance.
Pre-Employment Requirements
- Hiring is contingent upon completion of a background check, TB test, and clearance on a drug screen and pre-employment physical. Clean driving record and current first aid and CPR certification or willing to obtain upon hire. Additional requirements include receiving DOJ/FBI fingerprint clearance or an exemption and a submission of a signed Criminal Record Statement (LIC 508). Clean Background is a must.
- Montare Behavioral Health requires employees to be vaccinated with exceptions only as required by law (i.e. medical or religion).
