The LVN will play a crucial role in providing high-quality, personalized care to patients in their homes during challenging moments in their end-of-life journey. The ideal candidate will possess excellent clinical skills, a deep understanding of hospice care principles, and the ability to provide continuous care to patients and their families.

Responsibilities:

Continuous Care Provision:

      Deliver continuous nursing care to patients requiring more intensive support during critical periods.

      Administer medications, treatments, and interventions as prescribed by the hospice physician.

      Collaborate with the interdisciplinary team to develop and adjust individualized care plans.

Patient and Family Education:

      Educate patients and their families on the goals of hospice care and help them make informed decisions about treatment options.

      Provide guidance on symptom management and end-of-life care processes to enhance the overall understanding of the hospice journey.

Documentation:

      Maintain accurate and up-to-date patient records, documenting all care provided, assessments, and interventions in accordance with hospice policies and regulatory guidelines.

Compliance and Quality Assurance:

      Adhere to all relevant regulations, standards, and hospice policies to ensure the delivery of safe and effective care.

      Participate in quality improvement initiatives to enhance the overall quality of care provided.

Qualifications:

      Current LVN license in the state of practice.

Minimum of 2 years of experience in nursing, with a preference for hospice or palliative care experience.

      CPR certification and other relevant certifications as required by state regulations.

      Excellent communication and interpersonal skills.

      Ability to work independently and collaboratively within a team.

      Compassionate and empathetic approach to end-of-life care.

Schedule:

Per diem, flexible schedule based on the needs of patients.
