Social Worker discharge planner needed to work with various medical populations in a fast-paced hospital. Coordinate with RN case managers and other medical team members to develop safe discharge plans. Fast-paced units. hours 8-430p Must have LMSW and LCSW with recent hospital experience for the last 2 years.
