We are currently seeking a full time Licensed Social Worker to support the residents at our Continuing Care Retirement Community.

JOB RESPONSIBILITIES:

  • As necessary, begin interventions that will help the resident adjust to, and cope with, everyday living, problems, illness, transitions, crisis intervention, and/or end of life issues.
  • Maintain appropriate confidentiality, in compliance with HIPAA Law and as outlined in the Maryland State Board of Social Work Examiners Code of Ethics.
  • Actively participate in Resident Review Team (RRT) meetings
  • Coordinate support groups for residents & families as requested by the supervisor.
  • Have the availability to work flexible hours, when needed, including evenings and weekends.
  • Cover social work responsibilities in all levels of residency, as directed by the supervisor.
  • Provide Admissions Coordinator responsibilities which are listed below
  • Interview residents/guardians and obtain required information and signatures.
  • Admit and prepare identification records for residents in accordance with established policies and procedures.
  • Maintain an accurate record of available beds in Health Care Center and Assisted Living.
  • Admit, transfer, and discharge residents in accordance with established policies and procedures.
  • Obtain the resident/guardian's signature on all required permits, releases, authorizations etc.
  • Attach pre-admission documentation to admission papers as appropriate.
  • Collect, assemble and check admission papers. Forward to appropriate department.
  • Abstract information from records as authorized/required for insurance companies, Medicare, etc. following HIPAA guidelines.
  • Receive all pre-admission sign offs.
  • Maintain various registries as directed, including register for admission and discharge of residents.
  • Explain to the resident and/or guardian room rates, billing procedures, visitors/guest privileges, restrictions, resident care procedures, etc., as appropriate.
  • Refer admission problems to proper authority, i.e., office supervisor, credit manager, nursing service, etc.
  • Maintain daily and monthly census report as directed.
  • Maintain current records of all residents admitted/discharged by this facility in accordance with established policies and procedures.
  • Confer with nursing personnel and interdisciplinary team for assessments of potential admission candidates.
  • Is familiar with and complies with MD COMAR 10.07.14 and MD COMAR 10.07.02.
  • Identify and assure delivery of required psychosocial and mental health services in coordination with Consulting Psychiatrist.
  • Attend clinical supervision sessions conducted by the Social Work Director at least 3 hours per month.

QUALIFICATIONS:

  • Masters degree from an accredited college or university, including completion of a Social Work Program accredited by the Council on Social Work Education.
  • Current active license, in good standing, issued by the Maryland State Board of Social Work Examiners. Licensing must be maintained as a condition of employment.
  • The Social Worker will demonstrate an understanding of the aging process.
  • A high degree of interpersonal relations skills and capability of relating to a variety of people and personalities. Considerable initiative, judgment and leadership skills are desirable.
  • Ability to read, write, speak, and understand the English language.
  • Will demonstrate effective, empathetic methods of communication from a knowledge base applicable to the CCRC setting.
  • Ability to make independent decisions when circumstances warrant such action.
  • Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel, and the general public.
  • Ability and willingness to work harmoniously with other personnel.
  • Patience, tact, a cheerful disposition and enthusiasm as well as the willingness to handle difficult residents.
  • Ability to relate information concerning a resident's condition.
  • Will not pose a direct threat to the health or safety of other individuals in the workplace.
  • Long Term Care experience e.g. group home, regulated environment, dementia programming and family support.
