YOUR CONTRIBUTION TO OUR SUCCESS ESSENTIAL DUTIES & RESPONSIBILITIES:
  • Provide site based (on and off site) housing sustainability coaching and services to members.
  • Meet with members for individual consultations to address long term permanent housing and social needs; create Housing Sustainability Plan agreement in collaboration with the member and the Enhanced Housing Case Manager (EHCM) and update at least quarterly
  • Collaborate with the enhanced housing case management staff towards helping the member to achieve their housing sustainability plan (HSP)
  • Provide support and coaching to members towards achieving housing sustainability
  • Refer members to EHCM and Behavioral Health specialists for more needs that require intensive follow up, assistance and support.
  • Assist EHCM with coordinating educational programs for members that would help towards the goal of housing sustainability and stability. Topics should include but are not limited to workforce readiness, home ownership, budgeting, credit repair, applying for housing choice voucher program, mental health, Medicare/Medicaid, Social Security benefits, medical adherence, general health information, community resources, etc.
  • Create and compile housing options offered by property management staff, landlords, city agencies, community agencies and all other aspects involved in securing permanent
  • Compile and create educational information and resource materials in user friendly formats for distribution to members
  • Keep members informed about community resources, education programs, and general health information through an email distribution list
  • Link and advocate for housing entitlements and needed supportive services
  • Assist clients with completing relevant applications for housing subsidies and programs (HOPWA, non-HOPWA, Housing Choice Voucher Program, HUD, community ).
  • Locate, contact and follow through with potential housing resources and
  • Establish and develop new linkages with landlords/property management companies to increase the pool of affordable housing options for members. Maintain an active roster of
  • Cultivate relationship with existing permanent housing
  • Become an expert in subsidized housing
  • Track all referrals and provide on-going follow-up to clients and feedback to sources and
  • Advocate on behalf of the clients to ensure that they receive the appropriate subsidies
  • Perform timely and accurate input of all required client information in member chart (data entry and supporting documentation) to ensure compliance with regulatory and contractual Document all work done with and on behalf of members into agency and/or funder client database system in accordance with documentation standards.
  • Maintain client information and records in a confidential manner
  • Provide services to clients on an on-call (as needed) and on-going basis
  • Assess effectiveness of referral agencies and provide feedback to supervisor, as
  • Generate monthly program updates

Required Meetings/Trainings:

  • Meet with supervisor at least monthly to staff member cases, discuss chart audits/observation results, and to discuss performance and progress of stated goals.
  • Attend regular and mandatory trainings as assigned and required
  • Complete all Health Stream (online trainings) and required Member Services Trainings, HOPWA and HINT Specific Trainings annually or as assigned.
  • Attend program/department meetings as requested and required
  • Attend all required internal and external (for off-site) meetings.
  • Participate in various mandatory trainings held internally and externally ensuring a continuous knowledge base of programs that benefit members.
  • Attend agency, local, state, and national meetings, conferences, HOPWA HINT Project meetings, or workshops as needed or required.
  • Be a participant in the hiring and training of new staff as requested by manager.
  • Participate in and volunteer for intra and interdepartmental activities/events across the agency (i.e. AIDS Walk fundraising, agency sponsored events, member centered events, etc.).

QUALIFICATIONS: Education & Work Experience
  • High School diploma or equivalent, some college
  • Experience with social service or housing programs, knowledge of AIDS program development and non-profit service agency preferred.
  • Strong experience in assessing client needs for housing or other social services
  • Considerable skill in developing and maintaining databases and spreadsheets in Microsoft Excel
  • Experience in grant compliance, reporting and administration in a non-profit organization is preferred
  • Knowledge of the housing-related difficulties affected the well-being and health status of individuals with HIV
  • Familiarity with housing entitlement systems and
  • Excellent organizational, written and verbal communication
  • Ability to interact effectively with the client population and representatives of government and private
  • Ability to work effectively in a team
  • Willingness and ability to travel to potential housing sites, as
  • Bilingual – English/Spanish would be helpful.
