JOB SUMMARY:

The Registered Nurse Case Management Supervisor develops training curriculums, leads training, supports and provides consultations and assessments, as defined by DMAS, VDH, and other pertinent regulatory agencies for involved parties such as employees, contracted attendants, and individuals as it pertains to medical conditions, medications, procedures, and nurse, delegated tasks as needed or requested. This position must travel to required service locations, complete home visits and oversee nurse-delegated tasks.

SUPERVISION RECEIVED AND EXERCISED:

The Registered Nurse Case Management Supervisor will report directly to the Director of Agency Directed Services. The Registered Nurse Case Management Supervisor will be responsible for the supervision and management of the registered nurses and licensed practical nurses in the agency directed program.

Healthcare Operations

Assist with Healthcare Operations – 1) serve as a resource to program staff on medical issues or questions pertaining to individual's medical issues, 2) provide emergency on-call services on a rotational basis, 3) maintain and review all medical/health-related documents received (tests, lab work, doctor's notes, orders, diagnosis, treatments, medication, etc., as needed or requested by the Director; including the Admission Assessment information, and 4) provide initial and in-service training to providers.

Inspection Compliance

Inspect and ensure personal care attendants abide by 1) VDH and DMAS standards and guidelines of other pertinent regulatory agencies as applicable, 2) program policies and procedures and Human Rights, 3) nurse delegation, and 4) appropriate individual medical treatment.

Individual Assessment

Conduct observational assessments during 60-day (dependent on delegated services provided) home visits to assess individual's health & safety needs (e.g., psychosocial, nutritional, medical, education, physical injuries, etc.); included individuals not receiving nurse delegated services, and as requested by the Director.

Develop an Individual Service Plan per regulations to meet the individual's best interests appropriately; follow up with quarterly reviews to monitor ongoing needs and provide observations and input for PCP if necessary to satisfy the individual's best interests.

Documentation

Prepare and maintain new/received documentation (email, fax, home visits, phone calls, etc.) outlining program reporting & paperwork requirements related to individuals.

Admissions

Assist Agency Admissions by 1) meeting with potential attendants while ensuring individual's specific medical needs are being met appropriately, 2) attending initial attend initial admission placement

Training

Oversee training by 1) assisting the coordination of pre-service training program with trainers at service locations for providers (as scheduled by the Director) and 2) developing curriculums for involved parties (e.g., program staff) and yearly training schedule to allow for flexibility for the required service location(s); curriculums created as needed or requested.

Professional Development:

Complete required annual training(s) and participates in ongoing professional training and development to maintain up-to-date certification (CPR/First Aid) to assist service locations.

Reporting

Inform appropriate parties; immediate supervisor, CSB Support Coordinator, Care Coordinator, guardian, and in-service providers of significant events concerning the individual, such as; but not limited to, suspected abuse/neglect or serious incidents.

Participates in staff and regular supervision meetings, follows through with suggestions, and maintains a positive work environment with all involved parties, such as providers, parents, coworkers, referral agencies, community providers, all staff, and providers across DCR service locations.

Knowledge of:

· Office and records administration techniques and procedures

· The structure and content of the English language, including the meaning and spelling of words, rules of composition, and grammar

· Microsoft Office Word, Excel, PowerPoint, Outlook, SharePoint, Teams, and virtual meeting applications such as Zoom, WebEx, Google Meet

· DePaul's policies, procedures, and standard practices and the rules and guidelines established by regulatory and governing agencies such as VDH, and DMAS

· Relevant policies, procedures, and strategies to promote the safety and protection of people, data, and property

· Demonstrate through working knowledge of the principles of nursing and nursing skills including, but not limited to, assessment (medical, psychological, etc.), care planning, medications, medical case management, provision of sound therapeutic practice, and building relationships while maintaining professional boundaries

· Assist with the general upkeep and cleaning of office areas and agency vehicles

· Effectively work and consult with direct service staff and establish good working relationships with care providers/families, other community professionals, community service boards, and the general public

· Demonstrate the ability to interpret, understand and implement complex policies and procedures based on changing statutes and regulations

· Must possess excellent problem-solving and organizational skills with the ability to prepare and write effective reports, maintain records, plan, prioritize, and organize work

· Must be able to accept the rights, responsibilities, and differences of others

MINIMUM REQUIREMENTS TO APPLY:

A Registered Nurse certificate in good standing through the Virginia Board of Nursing is required. Previous supervisory experience is preferred.

WORK ENVIRONMENT:

The work environment characteristics described herein represent those an employee encounters while performing the position's essential functions. The employee regularly works in an office environment and occasionally in the field. Due to the nature of the work, the incumbent may be exposed to situations of a highly emotional nature; incumbents are expected to use verbal skills to resolve interpersonal situations to achieve desired results independently. Incumbents are exposed to various living situations and lifestyles. Work is generally completed regularly during weekdays; however, daily work hours will vary. Participation in an on-call rotation and completion of job duties outside of normally scheduled hours is required. Work is subject to frequent interruptions, and noise levels are varied based on the work locations.

PHYSICAL REQUIREMENTS:

The physical demands described herein are representative of those that an employee in this position must meet, with or without reasonable accommodations, to perform the essential functions successfully. Physical activities required are finger dexterity necessary to operate equipment used in the position, effective communication and seeing or hearing verbal cues. Walking for up to two-thirds of the time, sitting up to two-thirds of the time, bending/stooping for up to two-thirds of the time, lifting/pushing/pulling no more than twenty pounds, and minimal unassisted lifting associated with the job duties are required. When accessing various locations, the incumbent must climb stairs and ramps. This position requires driving approximately twenty percent of the time to various locations. SPECIAL REQUIREMENTS AND QUALIFICATIONS:

The following may be required after a conditional job offer: current DMV driving record with results that comply with insurer guidelines; drug and alcohol screening with satisfactory results stipulated by applicable governing, funding, or regulatory agencies; documentation of a PPD test, chest x-ray, or screening from a qualified medical professional indicating the absence of TB; criminal and child protective services background report.
