Implements treatment programs designed by the OT, collaborates with the OT to identify measurable functional outcomes. Implements occupational therapy procedures consistent in achieving identified goals, and grades activities to facilitate goals. Observes and reports pertinent information about participants to the OT. Provides regular updates to OT and Center Manager on status of participants in order to assist with care plan revisions and discharge planning of Elderhaus Day Center attendees and other agencies that provide care for Elderhaus participants. Participates as an active member of the interdisciplinary team and provides regular updates to team on participants’ status in therapy and reports any plans to discharge participants from therapy when assigned by OT. Assists in completion of documentation as directed by the OT. 8. Assists in developing special programs according to participant needs. Assists in developing and facilitating groups based on participant needs. Recommends ordering of durable medical equipment (DME) needs for participants. Reports issuance of DME to OT. Performs minor therapy equipment repairs as directed. Instructs other staff and/or caregivers on follow-through programs for participants when directed. Provides guidance to Aides in their delivery of services and reports as indicated to supervisor regarding pertinent issues. Provides assistance to Aides as needed. Consults with Recreational Therapy Department as directed in regard to activity adaptations.
