Inpatient Cardiac Rehabilitation Nurse

An independent, locally governed non-profit delivering premier comprehensive care is seeking an Inpatient Cardiac Rehabilitation Nurse to join their team in Naples, FL. The role includes assessing patient needs, developing educational plans based on goals and expectations, and facilitating the CABG discharge class. The ideal candidate would have a minimum of one year experience on a cardiology related patient care unit and possess extensive knowledge of cardiac disease, anatomy and physiology, risk stratification, exercise physiology, and diet and nutrition.

Responsibilities include:

  • Assessing the cardiovascular inpatient population to provide educational plans according to AACVPR guidelines
  • Identifying individual risk factors associated with cardiac and pulmonary disease and assisting with risk stratification
  • Developing individualized educational plan based on patient's goals and expectations, adjusting/reevaluating goals to meet the patient's needs
  • Facilitating the CABG discharge class in collaboration with other healthcare disciplines
  • Facilitating phase I to phase II transition process for cardiac rehab, actively assigning patients to phase II who qualify for the program and providing follow-up phone calls as necessary
  • Actively participating in patient education and community-based programs by organizing classes, communicating with speakers and participants, and providing actual classroom instruction

The candidate must have excellent verbal and non-verbal communication skills and be able to work and collaborate as a team member with nursing staff, medical staff, patient families, and other Cardiac Rehab personnel. A registered nurse license in the state of Florida and Basic Life Support (BLS) certification from the American Red Cross or American Heart Association is required.
