Job Summary:

We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Stratford Rehabilitation Center. As an LPN, you will play a vital role in providing high-quality patient care and contributing to the overall success of our healthcare team.

Responsibilities:

  • Assess resident's needs and develop individualized care plans
  • Administer nursing care and evaluate its effectiveness
  • Supervise nursing assistants and other personnel in the delivery of nursing care
  • Follow established health, sanitary, and infection control policies and procedures
  • Communicate effectively with residents, families, and healthcare team members

Requirements:

  • Current, unencumbered LPN license in the state of employment
  • CPR certification
  • Ability to communicate in English and follow verbal and written instructions
  • Good interpersonal skills and attention to detail
  • Ability to work with supervisors, co-workers, and community staff
