What You'll Do:
  • Deliver care in the patient's home through innovative in-home care service lines
  • Perform comprehensive health assessments, diagnose and treat complex illnesses
  • Provide therapeutic interventions and minor procedures
  • Use critical thinking skills and follow evidence-based standards
  • Analyze test data to diagnose, treat and arrange appropriate follow up
  • Educate patients and families on acute illness treatment and management
  • Communicate effectively with all individuals involved in the patient's care
  • Maintain positive relationships with team members
  • Adhere to clinical and safety standards
What You Need:
  • Minimum of two years of experience as an Advanced Practice Provider in acute care settings
  • Graduate with an advanced degree from an accredited institution
  • Current unrestricted state license and board certification
  • Current BLS required, ACLS certification preferred
  • Prescriptive authority and DEA
