Hospice On-call Registered Nurse

Responsibilities:

  • Provide end-of-life care and hospice services to patients in their home or community setting
  • Assess, plan, implement, and evaluate care needs during on-call hours in collaboration with the triage team
  • Report On-Call activities to the clinical leadership and Branch RN Case Managers at the end of the call shift
  • Maintain and complete appropriate care coordination notes for visit notes, and other pertinent clinical information
  • Admit patients eligible for hospice services in collaboration with the triage team under the physician's order
  • Develop patient care plan that specifically addresses identified patient problems during on-call hours, weekends and/or holiday(s)

Qualifications:

  • Current RN license in the state of practice or in accordance with the Board of Nurse Examiners rules for Nurse Licensure Compact (NLC)
  • At least two years recent experience as a professional nurse
  • Ability to meet physical demands and work independently
  • Knowledge of physical, psychosocial and spiritual needs of terminally ill patients and their caregivers
  • Effective written and verbal communication skills
