Responsibilities:

The Speech Language Pathologist will provide outpatient rehabilitation services primarily in an in-home setting for older adults. This may take place in a patient’s private home or in their room at an assisted or independent living facility. Powerback Rehabilitation To You empowers the Speech Language Pathologist to evaluate and treat patients in their most natural environment. Responsibilities include:

  1. Supervising Clinical Fellows, temporary licensees, students, and support staff as required by state licensure and professional standards.
  2. Adhering to clinical treatment schedules and ensuring timely documentation and billing.
  3. Participating in patient care meetings and contributing to a collaborative care approach.
  4. Referring patients to other services to ensure comprehensive care.
  5. Collaborating with rehabilitation team members and physicians to establish effective treatment plans.
  6. Adapting care plans to meet individual patient needs.
  7. Ensuring a safe environment by identifying and mitigating hazards.
  8. Ordering and maintaining necessary supplies and equipment.
  9. Delivering high-quality, compassionate service that meets the needs and rights of patients.
  10. Conducting assessments, designing treatment goals, and implementing speech language pathology services, which may include:
  • Cognitive-communication
  • Speech intelligibility
  • Oral motor skills
  • Swallowing disorders/dysphagia
  • Perceptual abilities
  • Memory
  • Functional communication
Coordinating the use of adaptive communication devices. Identifying hearing deficits and facilitating appropriate referrals. Selecting engaging activities suited to the patient’s capabilities and interests. Instructing patients in therapeutic activities, monitoring their progress, and adjusting the treatment plan as needed. Coordinating care with interdisciplinary teams and referring physicians. Performing any other related duties as required.

Qualifications:

  • Must have a valid Speech Language Pathologist license in the relevant state(s) or proof of eligibility.
  • Must possess or be eligible for a Certificate of Clinical Competence from the American Speech-Language-Hearing Association (ASHA) or qualify as an ASHA Clinical Fellow.
  • Must have a valid driver’s license and reliable transportation to patient residences.
