JOB SUMMARY:

As Regional Dietitian, you will be responsible for assessing the nutritional needs of our residents and developing individualized dietary plans to promote optimal health and well-being in multiple care locations. Working closely with interdisciplinary team members, you will play a pivotal role in ensuring that our residents receive nutritious and delicious meals that meet their dietary preferences, therapeutic requirements, and cultural preferences.

RESPONSIBILITIES:

  • Conduct comprehensive nutritional assessments of residents upon admission and as needed, considering medical history, dietary preferences, cultural factors, and therapeutic requirements
  • Develop individualized dietary plans and provide ongoing nutrition counseling to residents to promote optimal health, manage chronic conditions, and support rehabilitation and recovery
  • Collaborate with interdisciplinary team members, including physicians, nurses, therapists, and food service staff, to ensure coordinated and integrated care that meets residents' nutritional needs and goals
  • Monitor residents' nutritional status and dietary intake, and adjust dietary plans as needed based on changes in health status, treatment regimens, and dietary preferences
  • Provide education and training to staff members on nutrition-related topics, including special diets, food allergies, and feeding techniques, to ensure compliance with dietary plans and regulatory requirements
  • Participate in interdisciplinary care planning meetings, resident care conferences, and quality improvement initiatives to promote resident-centered care and enhance nutritional outcomes
  • Maintain accurate and up-to-date documentation of nutrition assessments, dietary plans, interventions, and resident progress in accordance with regulatory standards and facility policies
  • Stay abreast of current research, guidelines, and best practices in clinical nutrition and long-term care, and incorporate new knowledge and innovations into practice to optimize resident care and outcomes
  • Assist in policy review and development with focus related to nutrition services
  • Other duties as assigned
REQUIREMENTS:
  • Bachelor's degree in Nutrition, Dietetics, or a related field from an accredited institution
  • Registered Dietitian (RD) certification and licensure in the state of practice
  • Preferred experience working as a dietitian in a healthcare setting, preferably in skilled nursing or long-term care
  • Strong knowledge of medical nutrition therapy, therapeutic diets, and clinical nutrition principles
  • Excellent communication and interpersonal skills, with the ability to work effectively as part of a multidisciplinary team
  • Proficiency in nutrition assessment, dietary planning, and documentation
  • Commitment to providing compassionate, resident-centered care and promoting the dignity and well-being of our residents
