Job Duties:

Service Delivery Expectations:

1. Provides Senior PSR services to SMI participants to support and enhance the ability of the elderly to function at the highest possible level of independence in the most integrated setting appropriate to their needs.
2. Incorporates activities targeted to the specific needs and concerns of the elderly, while aiming to improve reality orientation, social adaptation, physical coordination, daily living skills, time and resource management, task completion and other areas of competence that promote independence in daily life.
3. Encourages member participation and tailors activities to accommodate different levels of cognitive/physical functioning.
4. Coordinates outreach activities and/or home visits to promote program involvement.
5. Maintains program census to meet service delivery expectations as established for fiscal year.
6. Maintains consumer confidentiality at all times.
7. Provides Senior PSR services in compliance with DMH/DOM standards.
8. Provides therapy as indicated in compliance with DMH/DOM standards (qualified staff only).

Documentation and Quality Improvement:

9. Collaborates with treatment team to develop individualized service plan.
10. Documents relevant events daily in appropriate case note format and completes monthly summaries as specified in DMH/DOM guidelines.
11. Completes and submits service and/or census logs per agency policy.
12. Maintains case record documentation that is adequate, appropriate, complete, and timely.
13. Works with treatment team to collect necessary information for PA submission. Assists with PA process as assigned. Ensures PA is received prior to program enrollment.

Interpersonal and Leadership Skills:

14. Provides direction, training and supervision for staff under direct supervision.
15. Accepts call-ins by staff and obtains adequate program coverage to ensure safety and compliance with standards.
16. Provides direction for PSR Program Assistants to ensure program compliance and consistency in the treatment program.
17. Maintains monthly evaluations of probationary staff under direct supervision.
18. Participates in the annual evaluation process of staff under direct supervision in collaboration with the County Administrator.
19. Attends and participates appropriately in clinical staffings. Recommends appropriate clinical services in addition to Senior PSR to meet the consumers' needs.
20. Responds to consumers needs in an appropriate and timely manner.
21. Demonstrates appropriate interactions with coworkers that promote a pleasant work environment.

Support Duties:

22. Completes additional job duties assigned by supervisor.
23. Participates in interagency collaboration for the provision of elderly services. Serves as liaison with local community agencies to develop resources for elderly program and community activities.
24. Notifies County Administrator a minimum of two hours in advance of any program absence so that the supervisor can arrange program coverage if necessary.
25. Monitors and orders supplies and equipment in a cost-effective manner while ensuring that the needs of consumers within the program are met.
26. Monitors the physical environment of the facility ensuring the cleanliness and safety of the environment at all times.
27. Pursues opportunities for supplemental funding to promote program continuation and success.
28. Signs out vehicles in compliance with CCS policy. Completes inspections prior to driving. Maintains safe driving habits. Ensures cleanliness of vehicles.

Key Responsibilities
ESSENTIAL QUALIFICATIONS/SKILLS/EXPERIENCE:

Bachelor's Degree in mental health related field required.
Requires excellent managerial, organizational, and interpersonal skills.
Requires satisfactory completion of CPR prior to reporting to the work site.
Requires satisfactory completion of CPI within the first 30 days of employment.
Requires ability to be covered under company insurance to drive a company vehicle for client transportation (community-based programs only)
Requires valid driver's license and current auto liability insurance.
Requires ability to transport clients in private vehicle when necessary.

PREFERRED QUALIFICATIONS/SKILLS/EXPERIENCE:

Previous experience in a mental health setting strongly preferred.
Previous experience working with an Elderly SMI population in a treatment setting strongly preferred.

Minimum Education
Bachelors

Minimum Requirements
Bachelor's degree in behavioral science or related field required. Master's preferred.

Licensure/Certification
Clinical licensure or DMH certification within 30 days of hire and maintained throughout duration of employment (master's level staff only)
