We are seeking a Registered Dietitian (RD) and Certified Diabetes Care and Education Specialist (CDCES), with at least one year of experience in diabetes education and management and in medical nutrition therapy. There is a solid preference for candidates who have diabetes pump and CGM experiences for devices currently on the market. Primary Responsibilities: Provide Diabetes Self-Management Education and Medical Nutrition Therapy as per provider referral Provide education via one-on-one consultation, groups, and either in –person or telehealth Provide appropriate documentation in Epic Work with the diabetes education team to develop a comprehensive diabetes device program Actively interact with physicians and staff, promoting a team approach to medical care Follow up on requests for diabetes or nutrition information from teammates and patients promptly via phone, email or MyChart Promote positive interpersonal relationships with fellow teammates, providers, physicians, and patients Attend monthly meetings for Certified Diabetes Care and Education Specialists You’ll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Bachelor's degree Registered Dietitian Certified Diabetes Care and Education Specialist (CDCES) 1+ years of experience in diabetes education and management and in medical nutrition therapy Knowledge of diabetes management and various diets for medical nutrition therapy including diabetes, pre-diabetes, congestive heart failure, hyperlipidemia, hypertension, GERD, gout, celiac, PCOS, chronic kidney disease Competent with Microsoft Office (Word, Excel, PowerPoint) Ability to read, speak, understand, and write clearly in English Preferred Qualifications: Master’s degree Device certification with Medtronic, Tandem, Omnipod 3+ years of experience in diabetes self-management education, medical nutrition therapy and diabetes management Experience using EPIC Skilled in establishing and maintaining effective working relationships with patients, medical staff and the public Solid teaching, counseling, and negotiation skills New York Residents Only: The hourly range for this role is $23.22 to $45.43 per hour.
