RESPONSIBILITIES

  • Supervise, orient, and mentor RN Case Managers
  • Ensure comprehensive discharge planning for patients and families
  • Identify and address gaps in services to improve patient care and resource utilization
  • Collaborate with multi-disciplinary teams, healthcare providers, and community partners

REQUIREMENTS

  • Associate Degree in Nursing required; BSN preferred
  • Minimum of 2 years supervisory/lead/manager experience in hospital Case Management
  • Registered Nurse license in Florida
  • Case Management Certification preferred
  • Basic Life Support (BLS) certification required
