Community Health, Case Manager II | Human Resources | Skamania County {rel="nofollow"} Under the clinical and managerial supervision of the Behavioral Health Manager. The Behavioral Case Manager Team Lead provides Behavioral Health services in both a community health department and community setting in collaboration with agency professionals in case planning and service coordination. [E]{.underline}[SSENTIAL FUNCTIONS: ]{.underline} Assesses eligibility, assists clients in accessing services. Assists with supervision of staff. Uses sound judgment to conforms to adopted policies and procedures, solves problems and achieves results consistent with stated goals and objectives. Works with the Behavioral Health Manager in the implementation of departmental programs and procedures to accomplish local, state and federal Behavioral Health goals and policies. Provides case management and health education services to all assigned clients. Services are provided in a variety of community settings, with consideration to client preference. Actively participates in joint case planning and coordination with internal multi-disciplinary team and agency psychiatrists. Utilizes agency clinical software system to prepare current client records and reports as necessary and required. Maintains timely and cogent progress notes and plans. Maintains proper and accurate documentation of all client and community encounters. Ensures that client records and other documentation are up-to-date and justify the case management services and community health worker services. Participates in meetings as required. Attends local, regional, and/or state meetings as requested by Managers and or Director. Within the scope of the position, performs duties independently in the community and promotes a positive image of the department and county. Provides coordination between behavioral health integrated programs and clinicians. Provide client transportation as needed. Coordinates appropriate referrals and follow-up beyond the scope of the behavioral health clinic's responsibilities and coordinates primary health care with patient's primary care provider. Evaluates program, client, and community needs within the context of the Community Health Department, the County, and the community. Applies appropriate professional process and Behavioral Health knowledge in response to individual behavioral health issues. Collaborates with others including health care providers, community partners, criminal justice, DSHS, schools, inpatient or residential facilities, and organizations to implement plans. Seeks consultation and collaboration when appropriate and or mandated. Practices within the scope of registrations, licensure, and or certifications. [KNOWLEDGE, SKILLS, AND ABILITIES: ]{.underline} Knowledge of all state and county requirements contained in the Revised Code of Washington (RCW) and Washington Administrative Code (WAC) or other directives to meet quality and record keeping requirements. Knowledge of modern supervisory practices and procedures. Knowledge and adherence to HIPAA as it specifically pertains to all Community Health programs. Knowledge of effective organization and prioritization of multiple program functions. Skill in following the overall strategy for the provision and allocation of behavioral health services. Skill in coping strategies to manage patient and documentation load effectively. Skill in supervising, evaluating and training employees.
