Job Duties

The MDS/Care Plan Nurse ensures timely interdisciplinary completion of the MDS/PPS assessments including RAPs (Resident Assessment Protocols) when appropriate in accordance with current federal, state, and local standards, guidelines, and regulations. This position is responsible for care management for all Medicare beneficiaries as well as completing supporting documentation accurately and in a timely manner. Candidate will be accountable for consistent documentation between MDS, nursing notes, care plans, MD notes and orders, therapy notes to obtain accurate reimbursement and new referral screenings for potential skilled admissions. This position also provides direct resident care as assigned.

The ideal candidate:
  • Current Registered Nurse licensure or Licensed Practical Nurse
  • MDS (Minimum Data Set) training & experience
  • Progressive nursing experience, preferably in resident assessment.
  • 3-5 years of long term care experience
  • Proficient computer skills; ability to utilize all Microsoft office applications and a host of other software applications as they apply to the position

Physical Requirements: Ability to stand and walk for extended periods; ability to lift and assist with resident transfers.
