Job Description
Northwestern Medicine has an exciting opportunity for either a Nurse Practitioner or Physician Assistant to work in our outpatient NMH Transitional Care Clinic.

Specifically, Transitional Care Clinic follow-up is for patients after an acute care crisis to help improve their health, understand how to navigate the medical system, and transition more easily to long-term primary care physicians in their communities (patients typically are followed in TC for approximately 6 months).
