Position Summary

The Transitions of Care Assistant is a core member of Lawndale Christian Health Center's (LCHC) Transitions of Care Team. The Transitions of Care Assistant will support the broader Transitions of Care team in supporting our patients who are currently or were recently hospitalized. The Transitions of Care Assistant is responsible for completing outreach efforts for emergency department visits or hospital admissions at community hospitals.

The Transitions of Care Assistant will be primarily based out of the Ogden location but may engage patients at local hospitals as determined by manager. When seeing patients at the hospital, the Transitions of Care Assistant is LCHC's representative there and plays a critical role in ensuring the smooth transition of care between the hospital and out-patient setting. The Transitions of Care Assistant must be proactive, persistent, well-organized, and function well in a fast-paced clinic environment.

Qualifications
  • Demonstrates excellent communication skills.
  • Displays independence and self-motivation; ability to initiate and achieve tasks without extensive supervision and feedback.
  • Strong ability to multi-task and work well in a fast-paced environment.
  • Requires experience and demonstrated abilities working in a multi-cultural setting.
  • Flexibility to work evenings and/or weekends if necessary.
  • Proficiency in excel and basic statistical analysis.
  • Embraces and personifies the mission of the Lawndale Christian Health Center, providing for the physical, emotional, and spiritual needs of staff members and patients.
Required Knowledge, Experience, or Licensure/Registration
  • B.S./B.A. in social/health sciences, nursing, or related degree required.
  • Bilingual Spanish preferred
