POSITION OVERVIEW

This position is located in the Gallatin City-County Health Department and performs duties that support the vision, mission and goals of the Department. This position is responsible for providing a variety of professional and administrative services within the health department to promote and maintain individual, group, and community health. This position will work with the support of a team of professionals/nurses to oversee the department's Ryan White program and services by providing medical case management and all the services that entails to ensure continued access to HIV medications and medical follow up for HIV positive individuals. This incumbent will coordinate participation in community health activities and provides presentations on health topics. Contact with the public in health care situations involves exposure to illness and disease. This position reports to the Director of Health Provision.

Each duty checked below makes up at least 20% of the job, and all other functions are included in other Duties as Assigned. All duties below are essential functions unless otherwise indicated.
A) HIV/AIDS Ryan White PROGRAM
  1. Provides medical case management services to eligible people living with HIV or AIDS according to requirements of the Ryan White Program.
  2. Ensures the Ryan White Program is utilized as a "payor of last resort" by helping clients apply for insurance (ACA, Medicaid, Medicare), apply for financial assistance, and utilize other low-cost community services to meet client needs.
  3. Collects data and prepares reports that document activities, services provided and expenditures.
  4. Collaborates and coordinates with other community agencies and resources to offer a variety of supportive and treatment services, based on the assessed needs of the individual client.
  5. Monitors and proactively facilitates continued access to HIV medication(s) and medical services to maintain/achieve viral suppression.
  6. Serves as a resource and referral contact for consultation on HIV disease, other common comorbidities, and Ryan White program services.
  7. Investigates circumstances, conducts interviews, and discusses sensitive personal information with those who have contracted or been exposed to HIV.
  8. Provides referrals for HIV treatment/follow up, and other associated services including housing assistance, eye/dental care, primary care, mental health and other assistance programs.
  9. Ensures compliance among clients within the context of program expectations and requirements.
  10. Works collaboratively with other health care professionals associated with HIV diagnosis, treatment and prevention.
B) COMMUNITY HEALTH
  1. Enhances community health through participation in a wide variety of local and countywide activities that promote partnerships, collaboration and education associated with effective public health resources and services.
  2. Teaches classes and makes public presentations on topics such as Ryan White program services, HIV prevention, treatment, and management.
  3. Collaborates with other organizations and agencies to provide comprehensive health services to individual clients, as well as develop population-based services.
  4. Participates on community boards and committees as a professional.
C) Other Duties as Assigned
  1. Perform a variety of other duties as assigned by supervisor. This includes coordinating special projects and events, system and grant research and analysis, representing the profession and the County at meetings, conferences, and special events, attending continuing education and training as required and accounting of time electronically at the end of the workweek.
  2. If the County Commission proclaims a local emergency due to an actual or threatened disaster such as an earthquake, fire, riot, flood, etc., County employees may be required to provide services during the emergency and for a subsequent period of assessment and recovery.

QUALIFICATIONS

This work requires knowledge of the principles and practices of public health including HIV/AIDS disease follow up and interventions, case management services, physical, family and community assessments, educational methods for individuals, groups and the public in general, public health laws and rules, community resources, record keeping, and basic computer skills. Also required is the ability to communicate effectively verbally and in writing; ability to prioritize tasks according to deadlines, work independently and follow instructions; ability to multitask and seek appropriate guidance when needed. The incumbent must have knowledge of all the Department's programs in order to provide assistance as needed. Must possess self-initiative but be able to work as part of a team. Position also requires customer service standards with a high-level of skill in dealing with the public, sometimes under stressful situations.

EDUCATION & EXPERIENCE

This position requires knowledge, skills, and abilities that are typically acquired through a combination of education and experience equivalent to graduation from an accredited college or university with advanced course work in social work, public health, or other social services or equivalent combination of education, training and/or experience. Experience in case management or social services, public health or a related area and basic Spanish language preferred.

ACCOUNTABILITY

This position is accountable for provision of direct services and carrying out standardized practices and procedures to a wide variety of problems or situations. Work may often require the incumbent to interpret and apply rules, regulations and policies to situations that may be without specific protocols and require individual professional judgment to determine the appropriate health services required. The incumbent in this position and the supervisor jointly determines and develop work objectives. Work Environment/Physical Demands

Work is performed in a wide variety of settings with clients and patients. Conducting vaccination clinics requires travel in inclement weather. Contact with the public in health care situations requires the incumbent to exercise universal precautions to prevent or minimize exposure to illness and disease. This position requires the incumbent to serve individuals and families who are under stress, mentally ill, or would be considered high-risk for other psychosocial reasons. At times, the incumbent would be required to handle contentious situations or angry public in a professional manner. The work requires moderate physical effort or exertion when carrying equipment or educational materials.

Excellent Medical Health Benefits - Group health benefits, including dental and vision coverage, are available to eligible County employees. The medical health benefits premium is covered for full-time employees. The County contribution is pro-rated on hours paid for part-time employees.
