As a Home Health Registered Nurse, your responsibilities will include:
- Providing admission, case management, and follow-up skilled nursing visits for home health patients.
- Administering ongoing care and case management for each patient, providing necessary follow-up as directed by the Clinical Manager.
- Coordinating appropriate care with various healthcare personnel.
- Reporting patient care/condition/progress to the patient's physician and Clinical Manager.
Required experience/skills:
- Diploma, Associate or Bachelor Degree in Nursing.
- Minimum of one year nursing experience preferred.
- Strong med surg, ICU, ER, acute experience.
- Current and unrestricted Registered Nurse licensure.
Scheduled Weekly Hours: 40
