Case Manager Supervisor

About the Position:

  • Full-time: Monday - Friday shifts
  • Location: New Mexico

We are seeking a highly motivated Case Manager Supervisor - RN who will be responsible for managing initial case reviews and triage of patients to case managers, as well as coordination of services for an organized, multidisciplinary approach. You will oversee department staff, act as a resource for patients and their families, and ensure compliance with appropriate policies and procedures. This position will require patient care assignment for various age groups.

Qualifications:

  • Bachelor's degree in related discipline
  • Current RN in New Mexico or through reciprocal agreement
  • 3 years of directly related experience
  • CPR for Healthcare/BLS Prov or Prof Rescuers within 30 days of hire

Bilingual fluency in English/Keres, Tewa, Tiwa, Towa, Zuni, or Navajo is preferred but not required.

Responsibilities:

  • Supervise department staff for efficient organizational structure and teamwork
  • Assess patients' needs and develop comprehensive multidisciplinary plan of care
  • Coordinate patient care to ensure effective, quality outcomes
  • Facilitate conferences to evaluate interdisciplinary dynamics and goals
  • Encourage patient and family participation for individualized care goals
