Position Summary:

Are you dedicated to making a difference in the lives of individuals and families in the community? CPLC Centro is seeking a compassionate and skilled Community-Based Clinician to join our team. In this role, you will provide vital mental health and behavioral services directly within the community, offering support to clients where they need it most.

As a Community-Based Clinician, you will manage a diverse caseload, conduct comprehensive assessments, develop and implement personalized treatment plans, and facilitate out-of-office services. You will also play a critical role in crisis intervention and work closely with community partners and stakeholders to ensure holistic care. This position requires a strong ability to work independently, excellent time-management skills, and a commitment to ongoing professional development.

Key Responsibilities:
  • Deliver direct and indirect mental health services to clients in community settings.
  • Manage and prioritize a caseload, ensuring timely and effective care.
  • Conduct thorough assessments and develop individualized treatment plans for clients.
  • Facilitate out-of-office services, including home visits and community outreach.
  • Provide crisis intervention services as needed, ensuring client safety and well-being.
  • Coordinate with community resources, supports, and stakeholders to enhance client care.
  • Participate in regular treatment team meetings, consultations, and ongoing learning opportunities.
  • Maintain accurate and timely documentation of client interactions and progress.
Minimum Qualifications and Competencies:
  • Education: Master's degree in a behavioral health-related field or currently enrolled in a Master's program.
  • Experience: At least two (2) years of full-time experience in behavioral health.
  • Licensing: Current license preferred, or the ability to become licensed.
  • Language: Bilingual in English and Spanish is required to effectively serve our diverse client population.
  • Certifications: Ability to obtain a fingerprint clearance card, CPR, and First Aid certification.
  • Driving: Valid driver's license with the ability to be insured under the agency's automobile policy.
  • Skills: Proficiency in MS Office (Word, Excel, Outlook, Access), excellent verbal and written communication skills, strong time-management, and organizational abilities.
  • Professionalism: Must maintain a professional appearance (business casual attire) and demonstrate excellent phone etiquette.
  • Work Ethic: Ability to work independently with minimal supervision, multitask, and be customer-service driven.
Preferred Qualifications:
  • Experience: Previous experience providing community-based mental health services.
  • Licensing: Licensed Clinician (LCSW, LPC, LMFT) is strongly preferred.
