Job Summary

The Palliative Nurse Practitioner is a Master's prepared nurse who acts as a consultant, educator, leader, clinical role model and nursing research resource in the specialty of palliative medicine. Demonstrates advanced knowledge, competency and skill in the diagnosis, treatment, and management of acute and chronic illness and disease. In addition, the APN has clinical privileges and performs delegated activities under direction of and in collaboration with a physician.

Population Served: Provides care to patients in need of palliative care in both inpatient and outpatient settings across ProHealth Care.

JOB ACCOUNTABILITIES

    • Identifies outcomes based on critical analysis of data relevant to patient, family and interdisciplinary/health care team with consideration of risks, benefits, burden and cost.
    • Manages and treats complex physical, emotional, social, and spiritual problems by utilizing research-based practice including pharmacological and non-pharmacological approaches.
  • Advocacy and Ethics: incorporates ethical principles and professional practice standards into care; advocates for the rights of patients and families.
  • Professionalism: demonstrates knowledge, skills, and attitude consistent with the advanced scope and standard of Hospice and Palliative Nursing Practice.
    • Performs duties in accordance with the ANA Code of Ethics for RNs, the Nurse Practice Act of the State of Wisconsin, and the related policies for ProHealth Care employees.
    • Serves as a role model, preceptor, mentor, and facilitator of learning in palliative care nursing and advanced practice hospice and palliative care nursing.
    • Maintains certification and licensure in accordance with institution, state and federal laws and regulations as well as the requirements of specialty nursing certification organizations.
    • Assumes accountability for palliative care nursing practice, assisting in the development of evidence-based practice standards, protocols and policies based on the synthesis of scientific literature.
  • Collaboration: promotes an interdisciplinary/health care team approach to providing advanced palliative care nursing
    • Consults with other disciplines and members of the interdisciplinary palliative care team in conduction patient/family conference to identify goals and plans of care.
    • Collaborates with other disciplines and members of the interdisciplinary palliative care team to ensure that the plan of care is understood by all ensuring continuity and consistency for patient and family.

EDUCATION/EXPERIENCE

Education: Master's Degree in Nursing with certification as an Advanced Prescriber Nurse Practitioner

Required licensure: Licensed Nurse Practitioner and Prescriber

Certifications: DEA, CPR. Preferred: Certification as an Advanced Hospice and Palliative Nurse

Minimum experience: 3-5 years as an APNP

Preferred experience: Background in palliative care, hospice, oncology or intensive care. Experience in pain management desired. Must be able to work independently and without direct supervision.

What You Will Do:

ProHealth Care is looking for an Advanced Practice Provider (NP or PA) who takes an active role in identifying and creating opportunities to educate, motivate and collaborate with staff and students in their professional development. PHC is looking for someone who renders advanced clinical practice services to all ages under a collaborative agreement with a designated PHMG physician. Responsibilities include: assessment, diagnoses, treatment and when necessary referral to others for further care.

Responsibilities
  • Assessment, diagnosis and treatment of patients under their care. Conditions managed include both acute and chronic illnesses with management of co morbidities in the ambulatory care setting.
  • Obtains medical histories and ensures relevant documentation in accordance with PHMG standards, the state of Wisconsin, PHMG Scope of Practice and collaborative agreement with PHMG physician.
  • Interprets and evaluates patient care data in order to determine management and treatment of diagnoses in accordance with evidence based practice, current clinical guidelines and medical processes. Orders and interprets diagnostic studies in the field of trained clinical specialty. Performs minor surgeries and routine procedures.
  • Encourages patient involvement in care through counseling, instructing and educating. Promotes active discussion with patient regarding plan of care, treatment options, care outcomes, preventive health measures and self-care skills.
  • Provides coordination of referral services including managing a variety of ancillary services, community resources and preoperative plans.
  • Prescribes, recommends and regulates both pharmacologic and non-pharmacologic therapies including controlled substances, as warranted for patient specific conditions.
  • Adjusts patient plan of care by monitoring treatment plans and patient outcomes.

  • Certification by the American Nurses Credentialing Center (ANCC) or American Academy of Nurse Practitioners (AANP) in Family Practice or Adult Primary Care
