Quality Director, Post Acute Care Services

MINIMUM QUALIFICATIONS:
EDUCATION, CERTIFICATION, AND/OR LICENSURE:
1. Associates degree in Nursing, Healthcare Administration, Business, or a related field.
EXPERIENCE:
1. 3-5 years of experience in quality improvement and data analysis.
2. Experience with regulatory compliance, including TJC and CMS requirements, is highly desirable

Analyze and manage clinical, financial, quality, utilization, and outcomes data to identify areas for improvement in patient care.
Lead quality improvement initiatives across multiple disciplines within home health and hospice services.
Ensure compliance with The Joint Commission (TJC), CMS, and other accrediting bodies by submitting required data in a timely manner.
Track progress on core measures and introduce new quality indicators to meet regulatory standards.
Develop and maintain software programs, spreadsheet macros, and database structures to support quality improvement projects.
Facilitate multi-disciplinary teams in using data to drive improvements in patient outcomes.
Serve as the administrator for HHCAPS, overseeing all regulatory aspects of home health and hospice services.
Update and maintain data on the Care Compare tool on a quarterly basis.
Conduct regulatory audits to ensure compliance with industry standards and guidelines.
Collaborate with internal stakeholders to develop and implement strategies for continuous quality improvement.
Maintain report writing capabilities on all relevant systems to support data-driven decision-making.
Provide guidance and support to staff in understanding and meeting quality and compliance requirements.
Monitor and report on the effectiveness of quality improvement initiatives, making recommendations for adjustments as needed.
Stay current with changes in regulatory requirements and best practices in home health and hospice care.
Ensure that all quality improvement activities align with organizational goals and regulatory requirements.
Lead the design, development, and execution of comprehensive training programs for home care staff, ensuring that all educational materials and sessions align with industry standards and regulatory requirements.
Provide leadership and guidance to the home care educators, setting performance expectations, conducting regular evaluations, and facilitating professional development to ensure the highest quality of training and education.
Oversee the onboarding process for new hires in home care services, ensuring that all staff receive appropriate orientation, training, and support to excel in their roles from day one.
Collaborate with the home care educator team to monitor and ensure that all training and onboarding processes meet or exceed internal quality standards and external regulatory requirements.
