Position Summary:

Under the direction of the Director of Ambulatory Care, the Ambulatory Care Manager is a key clinical leader supporting the operations of San Ysidro Health PCMH Care Teams. This role is critical, in partnership with medical leadership, for ensuring the implementation of the San Ysidro Health Model of Care designed to deliver safe high-quality care and improve health outcomes by meeting the population health management needs of the communities served.

The Ambulatory Care Manager directly manages the Ambulatory Care Supervisors (RN) and ensures appropriate training and resource support are provided for the delivery of licensed nursing and non-licensed clinical care at assigned outpatient clinics. This role identifies the population specific opportunities to increase health potential, improve health outcomes, and close gaps in care by monitoring provider, care team, and clinic performance data. The Ambulatory Care Manager is empowered, in collaboration with other leaders, to inform the San Ysidro Health Model of Care and portfolio of services to meet the needs of various populations represented.

Essential Functions of the Job:
  • In conjunction with Clinic Leads, facilitate coordinated care based on the patient centered SY Health model of care to meet organization patient safety and satisfaction, population health outcomes, and clinical quality goals; ensure staff provides the highest quality of care in compliance with the Nurse Practice Act, medical assistant scope, NCQA, TJC, federal, state and local requirements.
  • Work in collaboration with all members of the care team to support the healthcare needs of assigned populations through the continuous monitoring of data, trends identification, care team training, and implementing workflow efficiencies, pilots, and initiatives.
  • Based on sound process design principles, develop, implement, and monitor departmental protocols, policies, and procedures that support patient safety and satisfaction, effective care delivery, and achievement of organizational goals and objectives.
  • Support the development of clinic staffing standards and plans and identify roles and responsibilities of the Care Team Supervisor and other roles assigned.
  • Select, train, supervise, and develop Care Team Supervisors capable of managing assigned care team staff, oversee care team operations, and develop teams that meet the growing needs of the communities served.
  • Coordinates the annual continuing education schedule for assigned sites and ensures the clinical competency of each nurse and medical assistant prior to assignment of independent care activities and validates the clinical competency of nurses and medical assistants annually and as needed.
  • Analyze health data, and standard clinical performance metric set rates (UDS, HEDIS, and Utilization); consume and distribute reports to effectively manage the populations of assigned sites and inform monthly, weekly, and daily activities of the care team. Use this analysis to inform maturing the San Ysidro Health model of care through pilot, initiative, and program development.
  • Establish consistent communication mechanisms and regularly meet with care teams to identify and implement actions for improving population management outcomes.
  • Leverage the strengths of the San Ysidro Health spectrum of care to facilitate patient use of services to improve health outcomes and create connections that support patient navigation of the external healthcare system.
  • Maintain professional license and continuously develop professional knowledge base to include standards of clinical practice, regulatory changes, and industry innovations that extend the ability of care teams to meet established objectives.
Additional Duties and Responsibilities:
  • Assists with development and implementation of policies and procedures.
  • Trains various levels of staff.
  • Represents SY Health at various meetings and conferences.
  • Utilizes and organizes time efficiently to maximize productivity.
  • Works both independently and in a team-oriented, collaborative environment.
  • Adapts to shifting priorities, demands and timelines through analytical and problem-solving capabilities.
  • Ability to travel to multiple sites with SY Health network.
  • Performs other duties as assigned.
Job Requirements

Experience Required:
  • At least 5 years of clinical Registered Nursing experience with direct reports and/or unit oversight.
Experience Preferred:
  • Managed teams of clinicians, collaboration with different departments and other managers
  • A good grasp on how finances and budget work within an organization, cost breakdown and improving metrics.
  • Experience in reducing turn over, improving quality metrics across the board, developing staff training materials, and has previously led training for employees.
Education Required:
  • Graduate of an accredited registered nursing program
  • Associate Degree of Nursing (ADN)
Education Preferred:
  • Bachelor/Master's degree in nursing or health care related field.
Certifications Required:
  • Current California Registered Nurse (RN).
  • Driver's license with appropriate insurance coverage
Verbal and Written Skills Required to Perform the Job:
  • Excellent written and verbal communication skills.
  • Good interpersonal skills.
Technical Knowledge and Skills Required to Perform the Job:
  • Demonstrated high-level understanding of the PCMH model of care.
  • A strong ability to problem solve in a timely manner
  • Should have strong interpersonal skills with the ability to work efficiently and effectively with medical providers and support staff.
  • Comfort in using computer-based tools, processes, and workflows in performance job.

Working Conditions and Physical Requirements:
  • May include working long, and sometimes irregular hours, which encompasses working weekends.
  • Ability to sit and stand for extended periods of time. Driving and occasional travel required
  • Ability to be flexible, organized, motivated, multi-tasker and excellent time management skills.
Universal Requirements:
