Planning Coordinator Duties: The Planning Coordinator is responsible for maintaining all documentation for the individuals we serve as it relates to Life Plan Reviews, Staff Action Plans (SAP) and Individual Protective Oversight Plans (IPOPs). They will assist in creating sessions that foster active and healthy lifestyles, increase socialization skills, and connect with community resources. This position ensures compliance with regulations, agency policies and procedures. This includes family, local businesses, and external service providers. The Planning Coordinator will work collaboratively with others to ensure all services are provided with a person-centered approach to support each individual's health and well-being and will maintain on-going communication with all members of individual's team.

Planning Coordinator Qualifications:

High School diploma or equivalent required and 1 year of experience working with people with developmental disabilities.

Schedule: Monday-Friday 8am-4:00pm, with some flexibility

Salary Range: $18.25- $19.25 hr.
