Job Description

Harmony Village Coordinator

Location: CareOne at Livingston SNF (Livingston, NJ)

Hours: 9am-5pm Mon-Fri

Position Summary:
The Harmony Village Coordinator is responsible for the organization and management of the Harmony Program, with the exception of Nursing and Medical aspects of the Program. Works together with the Director of Nursing to assure the integrity of the program. This position will oversee both group and individual programs to ensure proper execution. The Harmony Village Coordinator will complete documentation, such as quarterly and annual progress notes, and individual visit and group activity attendance.
The Harmony Village Coordinator will also oversee all facility education. They will track monthly all education for the community, providing updates to all department heads on a monthly basis for education compliance. This position will display good time management skills, the ability to work both individually and as a team, will be punctual and professional, and self-motivated with a desire to increase each resident's quality of care, and overall quality of life.

Job Responsibilities:
-Maintain records of all employee monthly education.
-Provide monthly update to all department heads on staff education and follow up accordingly to
maintain compliance.
-Engage residents directly in programming throughout the day.
-Design and execute parallel programming to meet the varying needs of residents at different levels of
cognitive ability.
-Attend daily staff stand-up and Department Head meetings.
-Conduct periodic apartment checks and report all maintenance issues to Maintenance Director.
-Communicates to Health Services Director any Resident status change.
-Attends Resident care conferences including meetings with Hospice team.
-Ensures Resident Diet Board information is current and accurate.
-Ensures Residents' clothing and personal items are labeled.
-Assists with serving meals.
-Assists with housekeeping concerns as needed.
-Sets, maintains and communicates standards of performance to staff.
-Encourages staff participation in Resident activities.
-Develops and maintains excellent relationships with resident family members.
-Participates in family meetings and events
-Assists Executive Director and Health Services Director in addressing family concerns or questions as
appropriate.
-Participates in ongoing process of Quality Improvement to evaluate and improve upon services and
standards.
-Attends management meetings as required.
-Participates as a member of the Safety Committee
-Maintains confidentiality of residents' information and the Residence.
-Ensures excellent communication exchange with outside vendors.
-Performs any and all duties as assigned.

Reports To:
Directly reports to the Administrator of the facility where providing Harmony Program Services.
Recommended Minimum Position Qualifications:
Education
Must possess, as a minimum, a bachelor's degree in a health related field, or a current student in a health related program with experience in healthcare, nursing facilities, or with Alzheimer's and Dementia population.

Professional Experience:
Must possess some direct care experience within a healthcare field, may it be physical, emotional, cognitive, and/or social.
Exposure Risk:
The Harmony Village Coordinator can be at high risk for exposure to infectious waste, disease, conditions, etc., including TB, and Hepatitis B virus. Can also be at a high risk for exposure to assaultive adults and to blood and body fluid contact.
