This role requires a strong commitment to patient wellness, effective communication skills, and the ability to work collaboratively within a multidisciplinary team. *Duties* * Conduct comprehensive assessments of patients, including physical exams and health histories. * Develop and implement individualized care plans based on patient needs. * Manage patients with chronic conditions. * Ensure compliance with HIPAA regulations and maintain patient confidentiality at all times. * Educate patients and families on health management, disease prevention, and wellness strategies. * Participate in utilization management to optimize patient outcomes and resource allocation. * Adhere to infection control protocols to ensure a safe environment for patients and staff. * Collaborate with other healthcare professionals to coordinate comprehensive care. *Experience* Candidates should possess the following skills: * Strong background in patient care, particularly in wound care and critical care settings. * Proficiency in medication administration and knowledge of infection control practices. * Familiarity with behavioral health issues and treatment modalities. * We are looking for individuals who are passionate about improving patient outcomes while maintaining the highest standards of professionalism in all interactions.
