APRN for Nursing facilities (Melbourne, FL)Job description We are seeking a dedicated ARNP to join our healthcare team as a contractor. The ideal candidate will possess a strong medical background and a passion for delivering quality and comprehensive inpatient and outpatient care. Must be cognizant of, and comply with, all Center policies and procedures, as well as pertinent state and federal regulations. APRN must provide the full scope of pulmonary care services at nursing homes and long term care facilities, including but not limited to diagnosis, treatment, coordination of care, preventive care and health maintenance of their patients. The company provides Nurse- Practitioners to Skilled Nursing homes in Florida to work with the facilities nursing team to improve quality care and reduce rehospitalizations. The company is currently supporting Skilled Nursing Facilities (SNF) in the Miami, Broward and Tampa area, but other areas are also available.Responsibilities:Provide direct patient care in various healthcare settings, such as nursing homes and long term care facilitiesObtain health history and perform physical examinationsDiagnose and direct counseling for each patient on a plan for treatmentPrescribe medication in accordance with state statute and professional practice guidelinesMaintains a problem-oriented health record for each patientEvaluation of new-admission visitsCoordination of discharge visitsDevelop and implement patient management policies and proceduresCommunicate with attending physicians and/or specialists regarding patient careConduct routine check-ups to patients to assess their health condition and discover possible issuesGive appropriate advice for healthy habits (diet, hygiene etc.) and preventative actions to promote overall healthCultivate a climate of trust and compassion for patientsAssures continuity of care by providing comprehensive medical services on a timely basisProvides excellent internal/external customer service via telephone, videoconference, chat or face-to-face contact to assist patients with their health care needsFollows established departmental and medical center policies, procedures, and objectivesMonitor, assess, and document patient conditions and reactions to treatments.Collaborate with healthcare professionals to plan, manage, and assess patient treatmentsEducate patients and their families on proper healthcare proceduresEnsure compliance with infection control policies and proceduresAssist in medical procedures as neededSkills: The ideal candidate should have experience or knowledge in the following areas:Performing physical exams and patient observationsBehavioral health assessmentSetting up and monitoring medical equipmentCommunicating test results to patients and their familiesRecording patient medical historiesFamily planning servicesInfection control protocolsPost-acute care experience (Preferred)Senior care practicesMedical-surgical nursing techniquesStrong anatomy knowledgeThis role offers the opportunity to work in a dynamic healthcare environment, providing comprehensive care to patients across various specialties.
