Summary Evaluates and assesses patients to establish functional, measurable goals to promote restoration of functional abilities. Provides treatment according to a written plan of care to meet established goals. Plans for discharge and communicates findings and progress according to professional licensure/certification, scope of practice and departmental/discipline policies and procedures. Produces other reports as needed. May supervise clinical students and support staff including certified/licensed assistants, technicians, and volunteers. Adheres to the ASHA Code of Ethics. Provides a caring relationship that facilitates health, and healing. Performs other duties as assigned.Responsibilities + Evaluates and assesses the patient based on subjective and objective information.+ Communicates results of the evaluation to patient and other caregivers.+ Provides treatment according to established goals on plan of care.+ Documents interventions/care in the patient's medical record.+ Evaluates effectiveness of plan of care and reassesses patient's functional status as needed.+ Provides discharge planning information which includes patient/family education, follow-up, and referral to community resources.+ Contributes to the development of staff and students.+ Participates in activities designed to improve health care delivery.+ Completes assigned goals.Requirements, Preferences and Experience Experience Minimum: Entry level.Licensure, Registration, Certification Minimum: SLP;BLS CERTIFICATION WITHIN 14 DAYS OF HIRESpecial Skills Minimum: Basic computer literacy skills such as keyboarding, using a mouse, and navigating sites.About Baptist Memorial Health Care At Baptist, we owe our success to our colleagues, who have both technical expertise and a compassionate attitude.
