Or Master's Degree from an accredited Occupational Therapy Program. Or Ph.D. from an accredited Occupational Therapy Program. Or equivalent educ/experience Coursework/Training: Occasional specialized Occupational Therapy training to meet a particular department need. Upon hire: Oregon Occupational Therapist License National Provider BLS - American Heart Association upon hire.
