POSITION SUMMARY
The MDS Coordinator is responsible for coordinating all facets of the comprehensive resident assessment and the time completion of all areas of the MDS and PPS. Also interfaces with the Finance Department and conducts Performance Improvement activities related to the MDS process.
Qualifications:Requirements of the MDS Nurse (Registered Nurse):
- Must have prior MDS experience in a long-term care setting.
- Valid CT Registered Nurse (RN) License.
- Must have a current/active CPR certification.
- Excellent technical, assessment, and documentation skills.
- Effective verbal and written English communication skills.
- Highest level of professionalism with the ability to maintain confidentiality.
Responsibilities:
Duties and Responsibilities of the MDS Nurse (Registered Nurse):
- Initiating care plans and supporting activities as assigned
- Creating and distributing monthly care plan calendars in a timely fashion
- Maintaining & updating all care plans and assessments as required.
- Lead or participate in Daily PPS meetings, weekly Medicare meetings, and month end meetings to assure federal billing requirements are met.
- Information to complete the MDS is to be collected using the medical record, bedside assessment, and staff, resident and/or family interviews.
- Follow up on bloodwork, IV orders, and administration.
- Work collaboratively with the MDS vendor team on all aspects of the MDS process. (Including any potential audits).
- Join and update the nursing components of the weekly utilization review meetings.
