Responsibilities:
- Willing to travel for home visits.
- Develop individualized care plans in collaboration with healthcare teams.
- Monitor and evaluate patients' progress towards goals.
- Provide patient education and support to promote wellness and self-care.
- Document all assessments, care plans, and interventions accurately and timely.
- Collaborate with physicians, nurses, and other healthcare professionals to ensure coordinated and holistic care.
- Maintain up-to-date knowledge of healthcare policies, regulations, and best practices.
Requirements:
- Registered Nurse (RN) licensure.
- Proven experience as a Nurse Assessor or similar role in a healthcare setting.
- Strong clinical assessment and critical thinking skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and as part of a multidisciplinary team.
- Proficiency in electronic health records (EHR) systems.
- Compassionate and empathetic demeanor.
