Occupational Therapist (OT) Hearts of the Rockies Home Health Care LLC is seeking a dedicated and experienced Occupational Therapist (OT) to join our growing Medicare-certified Home Health Agency serving Pueblo and surrounding Southern Colorado communities. The OT will provide skilled, patient-centered occupational therapy services in the home, helping patients regain independence, improve safety, and perform daily living activities. Responsibilities include completing OASIS assessments (SOC, ROC, Recertifications, Discharges), conducting routine skilled visits, developing and managing Plans of Care (POC), and documenting all care in Alora Plus EMR, in compliance with Medicare, Medicaid, and Colorado regulations. The OT will collaborate with Physical Therapists, Speech-Language Pathologists, PTAs, COTAs, and nursing staff to deliver coordinated care, support patient functional goals, and participate in professional development and quality improvement initiatives.
